Croydon CCG and Croydon NHS Trust both placed in special measures – the CCG’s Clinical Chair responds

Croydon’s entire health service has been placed under new special measures designed to “restore financial discipline” to troubled parts of the NHS.

The borough’s NHS trust and clinical commissioning group (CCG) are among 14 providers forecasting “significant deficits” for 2016/17 that have been chosen by NHS England and regulator NHS Improvement to fall under the new regime.

Today, NHS Improvement and NHS England have announced a range of new measures to help address the financial challenges faced by a number of NHS organisations across the country. As part of this, Croydon CCG and Croydon Health Services NHS Trust have both been identified as requiring additional support to close our long-standing financial deficits.

For us, this follows NHS England’s 2015/16 Annual Assurance Framework where all CCGs have been given an annual rating.  Croydon CCG have received a headline rating for 2015/16 of requires improvement alongside 90 other CCGs across the country.*

As a result of our financial performance in the first quarter of this financial year, we have been put in financial special measures.  This means that NHS England will be providing us with extra support to work closely with our senior team to help further develop our financial recovery plan and take steps to help us to reduce our deficit.

As you know, Croydon CCG inherited a hugely challenging deficit which we have been working to address since our establishment.  We have also been historically funded to a lower level that we should have been to meet the health needs of our population. This means that money for healthcare in the borough has been particularly tight up until this financial year, as it is now close to the expected level.

We have clinically led savings of £72 million over the last five years and consistently met our financial targets whilst improving quality and performance each year.

We know even more needs to be done and we welcome NHS England’s continued support.   We have had independent assurance of our financial plans every year and we are under no illusions that this year will be our hardest yet.

Our focus is on transforming services to make them more efficient, effective and sustainable.  This includes:

  • Together with Croydon Council we are making great strides in our Outcomes Based Commissioning (OBC) programme of work, transforming the way services for patients over 65 years old will be provided in Croydon by putting the things that matter most to them and their families at the heart of service provision.
  • We are commissioning a new network of urgent care services, to improve the outcomes, safety and experience of local people, helping them to get the right treatment by the right person in the right place when it launches in April 2017.
  • We have made real improvements in cancer, mental health and A&E, urgent care and community services, improving the health of our population.

We are very grateful to our staff and GP membership for their hard work, support and commitment to finding new ways to make savings and efficiencies.  We need to focus our resources where we can have the biggest impact on people’s health and well-being whilst continuing to improve the quality of local services.

We will continue to address our financial challenges with our providers, Croydon Council and NHS England to make sure we do everything we can to bring our expenditure within our allocations.

NHS England’s 2015/16 CCG Assurance Framework has given each CCG a rating.  Croydon CCG have received a headline rating for 2015/16 of requires improvement alongside 90 other CCGs across the country.

As you can see from the five components of the rating, the financial position is the main element which has led to the overall rating.  For this element the CCG has been rated as inadequate, as has been the case since authorisation in 2012 primarily due to the inherited financial deficit.

The CCG are pleased to have been rated as ‘Good’ for both the ‘Well-led’ and ‘Delegated Functions’ element of our annual assessment.  The senior leadership team have made significant contributions to both the South West London STP and the Healthy London Partnership work for homelessness, 111 and urgency and emergency care.  NHS England also appreciate that we have demonstrated innovative thinking in the development of our Outcomes Based Commissioning work for older people in the borough.

The CCG’s ratings for ‘Performance’ and ‘Planning’ are understandably brought down by the financial position. Overall, NHS England recognises the CCG’s commitment and engagement, and that there are significant challenges in the local health economy as a whole.
